Register with the surgery

To register please bring your medical card to the reception desk. If you do not have your medical card let the receptionist know and she can give you the appropriate registration forms.

When you are registered with a GP your medical records will be transferred from your previous GP (it may take several weeks).

Where patients are requesting to join the Practice we do not discriminate on the ground of:

Race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

 

There are 2 registration forms for different circumstances in NI

  • Patients wishing to transfer GP within Northern Ireland or transferring from Great Britain need to complete a form HS200
  • Eligible visitors or persons taking up ordinary residence in Northern Ireland need to complete a form HSCR-1.

Paper forms are still available if you need one.

The service is designed and run by the NHS. It aims to cut practices’ workloads and make GP registration easier for the public.
